South Warwickshire University NHS Foundation Trust Shortlisted for HTN Awards 2024: Health Tech Case Study of the Year!
We are delighted to announce that South Warwickshire University NHS Foundation Trust (SWFT) has been shortlisted as a finalist for the HTN Awards 2024 in the category of Health Tech Case Study of the Year.
SWFT has earned its place as a finalist by leveraging Consultant Connect technology to revolutionise Frailty and Elderly Care Delivery. The Trust has successfully addressed challenges related to unanswered referral calls and unreliable service by providing clinicians with the ability to connect to specialists in less than a minute, making a significant impact on patient care.
The implementation began with the launch of the Care of the Elderly line in early 2017, giving GPs direct access to local frailty consultants for prompt advice. The service was set up using a rota of available consultants to answer calls, placed through the Consultant Connect app or dial-in number and all relevant data, call recordings, and activity reports were tracked on the secure Consultant Connect platform.
The technology has since been adapted beyond the initial requirements. In partnership, SWFT and Consultant Connect has collaborated with the West Midlands Ambulance Service (WMAS), the advanced clinical practitioner community team, and the Care of the Elderly team to establish quick two-way communication between teams.
The outcomes have proven highly successful, with 66% of calls from Primary Care to the Care of the Elderly resulting in the avoidance of hospital attendance and patients being directed to more suitable treatment locations.
