How We Can Help You Hit The CQUIN Milestones

shutterstock_151145456The Advice & Guidance indicator for the CQUIN is a significant opportunity for Acute Trusts to access a large sum of money. In exchange for implementing a simple service that requires comparatively little resource, Trusts can receive up to 0.25% of their annual contract value. For a Trust with a contract value of £400 million, that’s £1 million per year.

This article goes through the individual milestones that need to be achieved and how we can help you meet them. In fact, once the CCG / Trust leadership teams have agreed that Consultant Connect is how they will achieve some / all of the CQUIN, we can do most of the work required to trigger payments.


MILESTONE Q1 2017/18

Note: bullet lists are copied direct from the text of the CQUIN

  • Agree specialties with highest volume of GP referrals for A&G implementation (ACTION: Consultant Connect)
  • Agree trajectory for A&G services to cover a group of specialties responsible for at least 35% of GP referrals by Q4 2017/18 (ACTION: Consultant Connect)
  • Agree timetable and implementation plan for introduction of A&G to these specialties during the remainder of 2017/18 (ACTION: Consultant Connect)
  • Agree local quality standard for provision of A&G [including that 80% of asynchronous responses are provided within 2 working days] (ACTION: Consultant Connect)

Here are the actions that are required to meet the milestones

Consultant Connect will analyse local referral data (supplied by CCG or Trust) and make recommendations on which specialties to include in the CQUIN over the total 2 year duration of the CQUIN.

Consultant Connect will liaise with Trust management and clinical teams to understand local opportunities and constraints, identifying specialties that are ready to launch in the first year of the CQUIN.

Consultant Connect will agree with the Trust a final list of specialties that will be launched in 2017/18 and will draft an implementation plan and timetable in line with further milestones. Consultant Connect will submit this to the CCG for agreement.

Consultant Connect will produce a list of quality standards that are specific to and appropriate for the local area. In doing so, we will have regard to our national benchmark data, performance of other similar areas, local opportunities and constraints and any Advice & Guidance data that already exists for the area (whether Consultant Connect or other). Proposed quality standards will be submitted by Consultant Connect to the CCG for agreement.

MILESTONE Q2 2017/18

  • A&G services mobilised for first agreed tranche of specialties in line with implementation plan and trajectory (ACTION: Consultant Connect)
  • Local quality standard for provision of A&G finalised (ACTION: Consultant Connect)
  • Baseline data for main indicator provided (ACTION: Consultant Connect / Trust)

Following on from the Q1 Milestone, Consultant Connect’s Clinical Engagement Team will liaise with all specialty teams that have been nominated for launch in 2017/18. Clinicians will be offered meetings and documents to inform them about the service and prepare them for launch. It is possible this activity will take up to 6 months to complete, so will commence in Q1.

Quality Standards will hopefully have been agreed and finalised in Q1, but there is some flexibility here if needed and Consultant Connect will continue to liaise with stakeholders to conclude agreement in Q2 if required.

Baseline data is simply the total proportion of patient referrals that are to specialties that offer structured Advice & Guidance, and we are happy to help analyse the data if required.

MILESTONE Q3 2017/18

  • A&G services operational for first agreed tranche of specialties (ACTION: Consultant Connect)
  • Quality standards for provision of A&G met (ACTION: Consultant Connect)
  • Data for main indicator provided (ACTION: Consultant Connect / Trust)
  • Timetable, implementation plan and trajectory agreed for rollout of A&G services to cover a group of specialties responsible for at least 75% of GP referrals by Q4 2018/19 (ACTION: Consultant Connect)

Services will launch in line with the implementation plan, if not before, and Consultant Connect will keep both Trust and CCG updated with regular reports of readiness to go live across specialties. Prior to launch, Consultant Connect Clinical Engagement Team will work with GPs to prepare them for the initiative.

Once services have launched, Consultant Connect will monitor data on a weekly basis and report monthly to confirm that quality standards are being met.

Consultant Connect will produce a timetable and implementation plan for the remaining specialties to be launched. This will have been agreed in principle in Q1 but will be amended to take into account local opportunities / constraints that come to light. Consultant Connect will reissue the document to CCG, Trust and any affected specialties.

MILESTONE Q4 2017/18

  • A&G services operational for specialties covering at least 35% of total GP referrals by start of Q4 and sustained across the quarter (ACTION: Consultant Connect)
  • Quality standards for provision of A&G met (ACTION: Consultant Connect)
  • Data for main indicator provided (ACTION: Consultant Connect / Trust)

In Q4 Consultant Connect will provide a report for CCG and Trust confirming achievement of all milestones to date, performance to date, analysis of call data collected through the year and survey results from users. A review meeting will be held with all stakeholders to appraise project performance and to confirm the status of the project with respect to achieving future milestones.

ALL MILESTONES 2018/19

The majority of the work to get Advice & Guidance operational takes place in Q1 and Q2. The remaining milestones are more of the same: onboarding further specialties, monitoring quality standards and reporting:

  • A&G services in place for a group of specialties responsible for receiving 75% of total GP referrals by start of Q4 2018/19 and sustained across the quarter (ACTION: Consultant Connect)
  • Local quality standards met (ACTION: Consultant Connect)
  • Data for main indicator provided (ACTION: Consultant Connect / Trust)
